Yes — for programs over 90 days. US citizens can enter Spain (and the Schengen Area) visa-free for up to 90 days in any 180-day period. However, full-degree students or anyone staying longer than 90 days must obtain a Visado de Estudios (student visa) at the Spanish consulate before leaving the US. You cannot switch from tourist entry to student status inside Spain.
Generally no. US domestic health insurance plans (including BlueCross BlueShield, UnitedHealth, Medicaid, and most employer plans) are not accepted as sufficient proof of health coverage for the Spanish student visa. You need a Spanish or international policy that covers Spain, has no copayments/deductibles, and covers repatriation. Cigna International Student, Aetna International, Sanitas (Spanish), and Adeslas are commonly used by American students.
US federal student loans (FAFSA Direct Loans) are only available at Title IV-approved schools. Very few Spanish universities are on the Title IV list — IE University is one exception. For most Spanish universities, US federal loans cannot be used. Fulbright, private scholarships, and personal savings are the primary funding sources for most American students studying in Spain.
